Every insurance policy is different, but we can work with your insurance company to help you maximize your coverage. Please review our Notice of Privacy Practices, then complete these forms and fax them to Michelle Leassner, Bariatric Program Coordinator, at fax (314) 984-2795.

Cash

Our package price for the adjustable gastric banding procedure includes the price of the band, hospital charges and the surgeon’s fees. Anesthesia and radiology charges are billed separately.
